Trisha Francis, FNP-C, is a certified Family Nurse Practitioner and founder of Afaithful Hands Healthcare Services. She is dedicated to providing compassionate, patient-centered primary care to individuals in their homes, Assisted Living Facilities (ALF), Independent Living Facilities (ILF), and Memory Care communities throughout Southwest Florida.
With more than a decade of healthcare experience, Trisha has cared for patients across a wide range of settings including skilled nursing, home health, long term acute care, medical-surgical, hospice and palliative care. This diverse clinical background allows her to provide comprehensive care tailored to the unique needs of each patient.

Background & Experience
A Career Built on Caring for Those Who Need It Most
Trisha's healthcare career began as a Registered Nurse, where she gained extensive experience caring for patients in skilled nursing facilities, home health, long-term acute care hospitals, medical-surgical units, telemetry units, and emergency department observation settings. This diverse clinical background has provided her with a comprehensive understanding of the unique healthcare needs of older adults and individuals with complex medical conditions.
As a Nurse Practitioner, Trisha has specialized in primary care, palliative care, hospice care, chronic disease management, preventive medicine, and geriatric healthcare. She has cared for hundreds of patients across home-based, assisted living, independent living, and memory care settings — helping patients maintain their health, independence, and quality of life while receiving care in the environment most comfortable to them.
Prior to founding Afaithful Hands Healthcare Services, Trisha provided primary care services throughout Southwest Florida in assisted living, independent living, and memory care communities, She has also worked in palliative care and hospice medicine.
Trisha believes healthcare should be accessible, convenient, and personalized. Her mission is to build meaningful relationships with patients and families while providing high-quality medical care rooted in compassion, integrity, respect, and clinical excellence. Whether caring for patients at home, in assisted living, independent living, or memory care communities, Trisha is committed to helping every patient achieve their best possible health and well-being.
